Fitness Center Membership Application
The Underwood School Fitness Center is open to Underwood Community School District members for use if a membership is purchased. See the link below for two forms that need to be filled out or signed and returned to the Business Office or Matt McDonough with payment ($150 family and $90 individual adult). You will receive a key fob for use during community hours. Generally the center is available M-F mornings from 5:00-7:00 and M-F evenings from 6:00-10:00 (changes will be posted if school teams are using it). Weekend hours are throughout the day from 5:00 a.m. - 10:00 p.m. Memberships expire on August 1st of each new year. Renewal will be available at school registration and after.
It is required to have clean shoes, wear proper clothing, and keep the facility clean and organized. Adults must accompany school-aged kids at all times, no exceptions.
