Fitness Center Membership Forms
Fitness Center Memberships at the high school are $90 for a single member ($30 for a college student) and $150 for a family. Additional fobs are available for $10 each. Memberships run a year from the date of payment. Please fill out the forms below and turn them into the District Office at the middle school with the appropriate payment if interested. A key fob will be issued at that time. Forms only need to be filled out upon the start-up of your membership.
Hours for the Fitness Center are 5:00-7:30 A.M. and 4:00-9:30 P.M. on weekdays and 5:00 A.M. to 9:30 on weekends.
